Solution formulation. Most studies examining the efficacy of itraconazole solution have been in patients with impaired immunity. 13,55,56,57,58,59,60,61 Two large comparative studies with fluconazole (Table III) were in HIV-positive patients with oral (n = 244),57 or oropharyngeal (n = 190)58 candidosis: 14 days of itraconazole solution was at least as effective as fluconazole in effecting a clinical response (≥87%). In a further study of 126 immunocompromised patients with oesophageal candidosis, itraconazole solution and fluconazole led to a clinical response in 94% and 91% of cases, respectively.60 A comparative study examining the prophylactic use of itraconazole solution and fluconazole in 445 patients who were expected to be neutropenic following chemotherapy demonstrated that both agents prevent fungal infections in most cases (>97%).59

Peak plasma concentrations of itraconazole are reached within 2 to 5 hours following oral administration. As a consequence of non-linear pharmacokinetics, itraconazole accumulates in plasma during multiple dosing. Steady-state concentrations are generally reached within about 15 days, with Cmax values of 0.5 μg/mL, 1.1 μg/mL and 2.0 μg/mL after oral administration of 100 mg once daily, 200 mg once daily and 200 mg b.i.d., respectively. The terminal half-life of itraconazole generally ranges from 16 to 28 hours after single dose and increases to 34 to 42 hours with repeated dosing. Once treatment is stopped, itraconazole plasma concentrations decrease to an almost undetectable concentration within 7 to 14 days, depending on the dose and duration of treatment. Itraconazole mean total plasma clearance following intravenous administration is 278 mL/min. Itraconazole clearance decreases at higher doses due to saturable hepatic metabolism.



Its use in dogs, small mammals, and some exotics to treat fungal infections is 'off label' or 'extra label'. A similar statistically significant difference was also found in the average sizes of pellets among the brands with 300–500 pellets and those with <300 pellets. Brands 7 and 16 had unusually big pellets (1571 and 1845 μm) leading to very low pellet counts of 121 and 135, respectively.



Itraconazole was found to cause a dose-related increase in maternal toxicity, embryotoxicity, and teratogenicity in rats at dosage levels of approximately 40-160 mg/kg/day (5-20 times the MRHD), and in mice at dosage levels of approximately 80 mg/kg/day (10 times the MRHD). Itraconazole has been shown to cross the placenta in a rat model. In rats, the teratogenicity consisted of major skeletal defects; in mice, it consisted of encephaloceles and/or macroglossia.

And the treatment dosages and durations being given by many are much higher than the approved doses. Furthermore, use of 200 mg and higher dose capsule formulations (amply available in the Indian market) in this regard may produce suboptimal response. This is because keeping the same pellet size and technology, a 200-mg drug load formulation would require a much larger capsule size. The change in pellet size by altering the drug–polymer ratio or removing PEG 20000 layer leads to instable formulations with a negative impact on bioavailability.

Itraconazole showed no evidence of carcinogenicity potential in mice treated orally for 23 months at dosage levels up to 80 mg/kg/day (approximately 10 times the maximum recommended human dose ). Male rats treated with 25 mg/kg/day (3.1 times the MRHD) had a slightly increased incidence of soft tissue sarcoma. These sarcomas may have been a consequence of hypercholesterolemia, which is a response of rats, but not dogs or humans, to chronic itraconazole administration. Female rats treated with 50 mg/kg/day (6.25 times the MRHD) had an increased incidence of squamous cell carcinoma of the lung (2/50) as compared to the untreated group. Although the occurrence of squamous cell carcinoma in the lung is extremely uncommon in untreated rats, the increase in this study was not statistically significant.

24 March 2019 Heb begin 2017 diverse kuren achter elkaar moeten slikken en kreeg gigantische haaruitval, slapeloosheid, stoornissen van hart en bloeddruk, totaal energieloos, bedlegerig, psychoses en zelfmoordneigingen. 16 June 2019 Zit op dag drie nu, voel me een beetje alsof ik aan de speed zit en enorme hartkloppingen. Meerwaardige kat-ionbevattende geneesmiddelen zoals aluminium- of magnesiumbevattende antacida, sucralfaat, calcium-, bismut-, zink- en ijzerzouten en polymere fosfaatbinders (sevelameer, lanthaancarbonaat) verminderen de absorptie van ciprofloxacine; neem ciprofloxacine daarom ten minste 2 uur voor of ten minste 4 uur ná deze middelen in. Melkproducten of met mineralen verrijkte dranken (denk ook aan multivitaminepreparaten) kunnen de absorptie van ciprofloxacine eveneens verminderen.

Kuur van een week 2x 1,5 tablet.
Met name de uren na inname erg vermoeid, duizelig/misselijk, opgeblazen gevoel, zeurende hoofdpijn, spierpijn, zeer slecht en onrustig slapen, rillerig en koorts. Verschilt van moment tot moment. Paracetamol helpt deels.

Ik kreeg zelf het idee dat ik meer lichamelijke problemen had en terug moest naar de huisarts totdat ik hier de gedeelde reacties zag. Geeft iets meer rust. Even door de kuur heen komen!

If you are being treated for a urinary tract infection, call your doctor if you develop fever or back pain during or after your treatment. These symptoms may be signs that your infection is worsening.Take ciprofloxacin until you finish the prescription, even if you feel better. Do not stop taking ciprofloxacin without talking to your doctor unless you experience certain serious side effects listed in the

For the best effect, take this antibiotic at evenly spaced times.

 To help you remember, take this medication at the same time(s) every day.Continue to take this medication until the full prescribed amount is finished, even if symptoms disappear after a few days. Stopping the medication too early may result in a return of the infection.Tell your doctor if your condition lasts or gets worse.

Use of this medication for prolonged or repeated periods may result in oral thrush or a new vaginal yeast infection. Contact your doctor if you notice white patches in your mouth, a change in vaginal discharge, or other new symptoms.

This medication may rarely cause a severe intestinal condition due to a bacteria called C. difficile. This condition may occur during treatment or weeks to months after treatment has stopped. Tell your doctor right away if you develop: diarrhea that doesn't stop, abdominal or stomach pain/cramping, blood/mucus in your stool.

Symptoms and signs Aciclovir is only partly absorbed in the gastrointestinal tract. Patients have ingested overdoses of up to 20 g aciclovir on a single occasion, usually without toxic effects. Accidental, repeated overdoses of oral aciclovir over several days have been associated with gastrointestinal effects (eg nausea and vomiting) and neurological effects (eg headache and confusion). Overdosage of i.v. aciclovir has resulted in elevations of serum creatinine, blood urea nitrogen and subsequent renal failure.

 Neurological effects including confusion, hallucinations, agitation, seizures and coma have been described in association with overdosage. Treatment Patients should be observed closely for signs of toxicity. 

Haemodialysis significantly enhances the removal of aciclovir from the blood and may, therefore, be considered a management option in the event of symptomatic overdose.

This medication may rarely cause a life-threatening disorder that affects the blood cells, kidneys, and other parts of the body. This disorder is more likely to occur if you have conditions related to a weakened immune system (such as HIV disease, bone marrow transplant, kidney transplant). 

Get medical help right away if any of these very serious side effects occur: extreme tiredness, slow/fast/irregular heartbeat, easy bruising/bleeding, new fever, bloody/dark urine, severe stomach/abdominal pain, yellowing eyes/skin, sudden vision changes, loss of consciousness, seizures.

Pregnancy The use of aciclovir should be considered only when the potential benefits outweigh the possibility of unknown risks. A post-marketing aciclovir pregnancy registry has documented pregnancy outcomes in women exposed to any formulation of aciclovir. The registry findings have not shown an increase in the number of birth defects amongst aciclovir exposed subjects compared with the general population, and any birth defects showed no uniqueness or consistent pattern to suggest a common cause.

 Systemic administration of aciclovir in internationally accepted standard tests did not produce embryotoxic or teratogenic effects in rabbits, rats or mice. In a non-standard test in rats, foetal abnormalities were observed but only following such high subcutaneous doses that maternal toxicity was produced. 

The clinical relevance of these findings is uncertain. Caution should however be exercised by balancing the potential benefits of treatment against any possible hazard. Findings from reproduction toxicology studies are included in Section 5.3. Breast-feeding Following oral administration of 200mg aciclovir five times a day, aciclovir has been detected in breast milk at concentrations ranging from 0.6 to 4.1 times the corresponding plasma levels. These levels would potentially expose nursing infants to aciclovir dosages of up to 0.3mg/kg/day. Caution is therefore advised if aciclovir is to be administered to a nursing woman. Fertility There is no information on the effect of aciclovir on human female fertility. 

In a study of 20 male patients with normal sperm count, oral aciclovir administered at doses of up to 1g per day for up to six months has been shown to have no clinically significant effect on sperm count, motility or morphology. See clinical studies in section 5.2.

Aciclovir is only partially absorbed from the gut. The average oral bioavailability varies between 10 and 20%. Under fasting conditions, mean peak concentrations (Cmax) of 0.4 microgram/ml are achieved at approximately 1.6 hours after a 200 mg dose administered as oral suspension or capsule. Mean peak plasma concentrations (Cssmax) increase to 0.7 microgram/ml (3.1 micromoles) at steady state following doses of 200 mg administered every four hours. A less than proportional increase is observed for Cssmax concentration following doses of 400 mg and 800 mg administered four-hourly, with values reaching 1.2 and 1.8 microgram/ml (5.3 and 8 micromoles), respectively.

Continue to take this medication until the full prescribed amount is finished. Do not change your dose, skip any doses, or stop this medication early without your doctor's approval.
